Please note: Before applying for a badge verify with your departments' badge coordinator or Patty Pschierer to determine whether your department utilizes badges and has an assigned badge series.

Requesting A Badge -If indeed you need to have a badge in your lab/department you can download the 5C Radiation Worker Form and complete it.

How much will a badge cost? You can find all badge fees listed on the RSU Fees page.

I'm pregnant -do I need to declare my pregnancy? Pregnancy declaration is voluntary. You do not need to declare and you may not even need a badge in your department. However, if you wish to monitor the potential exposure during your pregnancy you should contact our office, as early as possible. Or you can also contact our office to schedule a short meeting with the Radiation Safety Officer to simply gather information. Even after an informational meeting you still do not need to declare your pregnancy if you do not wish to.
